At Valour Health Services, our community nurses provide professional support to help veterans understand and manage their medications as part of their overall healthcare plan. This may include reviewing medications, assisting with medication schedules, and monitoring for any potential concerns.

Accessing This Service Through DVA

Community nursing assessments may be available to eligible veterans through the Department of Veterans’ Affairs (DVA) Community Nursing Program. In most cases, a referral from a General Practitioner (GP) or healthcare professional is required before services can begin. Once a referral is received, our nursing team will arrange an assessment to determine the type and level of care required. If you are unsure about eligibility or how to begin the referral process, our team at Valour Health Services is happy to provide guidance and assistance.
