Health Monitoring

Regular monitoring of vital signs and general health to help identify changes early and maintain wellbeing.

About Health Monitoring Service

Regular health monitoring plays an important role in maintaining wellbeing, especially for individuals managing ongoing health conditions or recovering from illness. Routine checks help identify changes in health early and allow appropriate action to be taken when needed.

At Valour Health Services, our community nurses provide professional health monitoring services in the comfort of the client’s home. During visits, our nurses may check vital signs such as blood pressure, pulse, temperature, and other health indicators depending on the individual’s needs.

By providing regular monitoring and professional guidance, we help veterans stay informed about their health while supporting early intervention when concerns arise. This proactive approach helps maintain stability, prevent complications, and support independent living at home.

Who is this service for?

Veterans who require regular monitoring of vital signs and general health
Individuals managing chronic health conditions
Veterans recovering from illness or medical treatment
Individuals whose health would benefit from regular nursing check-ins
Veterans who require reassurance that their health is being monitored
Families seeking professional oversight of a loved one’s wellbeing

Accessing This Service Through DVA

Community nursing assessments may be available to eligible veterans through the Department of Veterans’ Affairs (DVA) Community Nursing Program. In most cases, a referral from a General Practitioner (GP) or healthcare professional is required before services can begin. Once a referral is received, our nursing team will arrange an assessment to determine the type and level of care required. If you are unsure about eligibility or how to begin the referral process, our team at Valour Health Services is happy to provide guidance and assistance.
