Community nursing assessments are an important first step in understanding a veteran’s health needs and determining the most appropriate level of care. These assessments are carried out by qualified nurses who evaluate a client’s overall health, medical history, mobility, and daily living needs.

At Valour Health Services, our nurses conduct assessments in the comfort of the client’s home. This allows us to gain a better understanding of the individual’s living environment and any factors that may influence their health or safety.

Accessing This Service Through DVA

Community nursing assessments may be available to eligible veterans through the Department of Veterans’ Affairs (DVA) Community Nursing Program. In most cases, a referral from a General Practitioner (GP) or healthcare professional is required before services can begin. Once a referral is received, our nursing team will arrange an assessment to determine the type and level of care required. If you are unsure about eligibility or how to begin the referral process, our team at Valour Health Services is happy to provide guidance and assistance.

Initial Health Consultation
Our nurse meets with the client in their home to discuss their health history, current conditions, and any concerns they may have.
Comprehensive Health Assessment

Home Environment Review
