Returning home after a hospital stay can be a challenging time, especially when ongoing care or monitoring is required. Post-hospital care provides professional nursing support to help veterans recover safely while adjusting back to daily life at home.

At Valour Health Services, our community nurses assist with monitoring recovery, managing treatment plans, and ensuring that discharge instructions from the hospital are followed correctly. Our goal is to provide reassurance and support during the transition from hospital to home.

Accessing This Service Through DVA

Community nursing assessments may be available to eligible veterans through the Department of Veterans’ Affairs (DVA) Community Nursing Program. In most cases, a referral from a General Practitioner (GP) or healthcare professional is required before services can begin. Once a referral is received, our nursing team will arrange an assessment to determine the type and level of care required. If you are unsure about eligibility or how to begin the referral process, our team at Valour Health Services is happy to provide guidance and assistance.

Our nurses review discharge instructions and treatment plans to ensure the client understands their recovery requirements.
